The key is to dial in the bitrate - you'll need about 800kbps video with 100kbps audio to hit 200MB. That's low for for SD resolution, but it might be good enough for you. Also look for software that has an option for 2-pass encoding.

Be aware that you are not changing anything. If I call you Bob, you are still Jac and even then you are probably posting under a different name. If you have software that naively trusts the filename instead of examining the actual binary headers of the file, this can work for you. If the actual format or encoding is wrong then no name change will fix that.

Yes, this simple conversion should work fine. Both files are technically mp4 files anyways.

Avidemux should do the work.
Load File
Go to File -> Append and select File
To the left of the video window,
set video mode to Copy;
same for audio mode;
set format to MP4.
Go to File -> Save -> Video.

You can use Avidemux or Yamb to separate audio and video tracks, then convert the CBR to VBR audio and use avidemux to join them together.

I found that you get better results from gradients in AE when you run in floating point, i.e. 16 or 32bit and output those very bits to the encoder. This means not introducing banding by downsampling it to 8 bit before sending it to the encoder, or even the internal Adobe encoder in AE (which is crap for low bitrates).

If you are using the Ramp effect then you can increase the Ramp Scatter value which helps decrease banding.
Also working in 16 bit or 32 bit rather than 8bit can also help!
